EXCELLENT  Near mint. Almost no signs of use. Lens is mint if included.

GOOD+  Minor external wear. Lens has no fungus, haze, or scratches.

GOOD  External wear from normal use. Glass may have internal dust or small cleaning marks, will not affect an image.

GOOD-  Rough looking. Glass may have imperfections. Fully functional.

UGLY  Dents, scratches, heavily used. Glass may have imperfections that affect image quality.
